Renewal Application Form Checklist
All applicants must submit:
- Completed renewal application form
- Licence Renewal fee
- Supporting information relating to self-declaration questions (if applicable)
- Criminal Record and Judicial Matters Check for each new individual principal, director, and officer of the business (conducted within 6 months from application submission, via HCRA’s third-party provider or original report from a municipal police service or the Ontario Provincial Police).
- HCRA Notice of Consent Form completed by each new individual principal, director, and officer of the business.
- Photo ID (i.e., driver’s licence or government issued identification) for each new individual principal, director, and officer of the business.
- Any other appropriate documents related to any changes to the licence or to the Principals, Directors, Officers, or Employees listed on the licence (Refer to the New Application Checklist for details of required documents, etc.).

Note: You will receive a confirmation email after submitting your application. If you don’t receive a confirmation email, check that you have clicked “submit” after completing your payment. If you do not receive the confirmation email and you have submitted the application, contact customer service for assistance at info@hcraontario.ca or 416-487-4272.
